Description
Create new WordPress setup -> upload the plugin, install/activate -> create new topic/view topic -> Page not found... :(
Why every time we should save the permalinks after the plugin install?

Fixed this a while ago, though still not confident it will work correctly in all circumstances. It's a catch 22 situation, but I think I've improved it as much as it can be until changes are made in WordPress core to accommodate for this.

Ironically enough, there is one already. Because of how plugins are activated, it doesn't seem to always get fired correctly. I agree this is an issue, but probably not one to fix for 2.1.3.
Moving to 2.2 to pick it up later.
